WORKSHOP PARTICIPATION & MEDIA RELEASE

1. Introduction

This agreement is between jazziesmith.com and the participant.

2. Agreement to Participate

By registering for and attending any workshop offered by jazziesmith.com (the "Workshop"), you ("Participant") agree to the terms outlined in this Workshop Participation & Media Release.

3. Consent to Record

Participant understands the workshop will be recorded (audio/video).

4. Recording and Documentation

Participant acknowledges and agrees that:

• All Workshops may be recorded via audio and/or video

• Recordings may include Participant's voice, image, likeness, and any questions or comments made during the Workshop

• A replay of the Workshop will be made available to registered participants

• Participant's participation constitutes consent to be recorded

5. Media Release

Participant grants jazziesmith.com the right to use their voice, image, and comments from the recording for future workshops, marketing, and products without compensation.

6. Media Release and Usage Rights

Participant grants jazziesmith.com and Jazzie Smith a perpetual, worldwide, royalty-free license to use, reproduce, distribute, and display recordings of the Workshop, including Participant's voice, image, likeness, and contributions, for the following purposes:

• Promotional and marketing materials

• Social media content

• Future paid or free educational courses and workshops

• Website content and testimonials

• Email marketing campaigns

• Training materials for other hypnotherapists

7. No Compensation

Participant understands that no compensation, monetary or otherwise, will be provided for the use of their likeness, voice, or contributions in Workshop recordings or related materials.

8. Participation

Participant agrees to maintain a respectful environment.

9. Waiver and Release of Liability

Participant agrees to release and hold harmless jazziesmith.com, Jazzie Smith, and their representatives from any claims, damages, or liabilities arising from:

• Participation in the Workshop

• Use of Workshop recordings and materials

• Any hypnotherapy techniques or information shared during the Workshop

Participant acknowledges that Workshop content is for educational purposes and is intended for licensed or trained hypnotherapy professionals.

10. Disclaimer

Hypnotherapy is for educational/wellness purposes and is not a substitute for medical advice.

11. Professional Responsibility

Participant acknowledges that they are responsible for determining whether Workshop techniques and information are appropriate for their practice and clients. jazziesmith.com does not guarantee specific results from implementing Workshop teachings.

12. Privacy and Confidentiality

While jazziesmith.com will make reasonable efforts to protect Participant privacy:

• Participant names may appear in recordings or transcripts

• Participant contributions during the Workshop may be shared publicly

• Participants should not share confidential client information during Workshops

13. Opt-Out Limitations

Once a Workshop recording has been edited and published, removal of individual Participant contributions may not be feasible. Requests to opt-out of media usage must be submitted in writing to hello@jazziesmith.com within 48 hours of the Workshop conclusion.

14. Intellectual Property

All Workshop content, materials, recordings, and techniques remain the intellectual property of jazziesmith.com and Jazzie Smith. Participants may not record, reproduce, or distribute Workshop content without express written permission.

15. Modification of Terms

jazziesmith.com reserves the right to modify this agreement at any time. Continued participation in Workshops after modifications constitutes acceptance of updated terms.

16. Governing Law

This agreement shall be governed by the laws of the State of Colorado, United States of America.

17. Agreement

By booking via TidyCal and checking the agreement box, the participant agrees to these terms.

18. Acknowledgment

By registering for and attending the Workshop, Participant acknowledges that they have read, understood, and agree to be bound by this Workshop Participation & Media Release.
